Community Options provides short or long term case management for people who are frail aged,

people with a disability [and their carers] to help them stay in their own home. They can

organize and coordinate a range of home and community services as well as preparing r

eferrals to medical services, financialcounseling and personal counseling services



ComPacks provides up to 6 weeks of co-ordinated services to assist a person returning safely to

home after a recent hospitalisation in a participating NSW public hospital. Referrals are required

by hospital staff.

Telecross & TeleCHATTele-chat

Telecross is a service run by the Red Cross to assists the elderly and housebound in their own home.  Every day a trained Red Cross Volunteer makes a short telephone call to the Telecross client to ensure that they are safe and well.  If the call is not answered, a second call is made.  If this call goes unanswered an agreed emergency procedure will be activated and help arranged if necessary.

Telecross can be provided on a temporary basis, during a period of rehabilitation or when a carer is absent. People can refer themselves or be referred by a relative, friend or health professional.

Red Cross also provides a sevice called TeleCHAT; regular telephone calls to people who have little or no social connection with other people. Friendly volunteers call participants at an agreed time, generally once a week, for an open ended chat. Participants are matched with volunteers based on their interests, hobbies and availability.
